Treatment of pathological fractures or impending fractures should be based on a multidisciplinary approach. Surgical procedures have to be suited not only to local mechanical conditions but also to patient’s life expectancy and behaviour of the particular tumor in relation to adiuvant not surgical treatments. Indications of osteosynthesis versus resection-reconstruction, choice of fixation alone or after curettage, different fixation options in metastatic disease and technical tips are analyzed.
