Objectives: after outlining the new scenario that the emergence of the “Medicine of the 4 P” entails for the participation of citizens in healthcare, in particular with regard to the so-called “Participatory Medicine”, a model of multi-level partnership is presented aimed at a sociologically comprehensive approach articulated at the three micro-meso-macro levels and potentially able to obviate the risks that the rhetoric of empowerment can entail. Methods: on the basis of a historical reconstruction of the evolution of the participation system of the Tuscany Region in the health sector, the project of participatory training-research-action in progress is then presented, aimed at favoring organizational self-learning processes and strengthening the stewardship role of the public institution in community governance processes for health. Results: the project of participatory training-research-action is currently being implemented in the forms of multi-level partnerships implemented in the Tuscany Region described, the results of which will be interesting to analyze at the end of the experimentation.
