If the re-design of decision-making processes in a participatory direction represent one of the main challenges in the welfare systems since the 1970s, it is with the new millennium that we are witnessing a growing protagonism of the Public Authorities in this field. It is a leading role heralding ambivalent outcomes, which on the one hand expresses the attempt to maintain its role as guarantor of fundamental rights but which, on the other hand, can lend itself to the processes of de-responsibility of the Public Authority. Twenty years after the reform of Title V of the Constitution, this paper explores and compares the institutional representations of the role of citizens-users / patients in five Italian regional health systems. Our empirical research, carried on through the Delphi technique, highlight the instruments currently in use and explore the impact of the pandemic, developing some critical considerations on possible future scenarios of the regionals models.
