INTRODUCTION: Ulcerative colitis (UC) is a chronic inflammatory bowel disease (IBD) often requiring immunomodulatory therapy. This report presents a case of severe pneumonia in a UC patient treated with infliximab, an anti-tumor necrosis factoralpha agent. It highlights the diagnostic challenges in distinguishing between pulmonary extra-intestinal manifestations (EIM) of UC, infectious or autoimmune-related pneumonia, and drug-induced organizing pneumonia (OP).
