This demo presents a multi-modal system for the remote, continuous, and non-intrusive monitoring of patients with Chronic Obstructive Pulmonary Disease (COPD), developed within the framework of the European project TOLIFE. The proposed architecture integrates six complementary heterogeneous sensing devices: a smartphone, a smartwatch, a portable spirometer, smart shoes, a smart mattress cover, and a monitoring environmental unit. This kit collects heterogeneous physiological and contextual data in a continuous manner with minimal interference in the patient's daily activities. Data are offloaded to a collection and analysis platform based on InfluxDB, supporting scalable time-series data storage and retrieval. A web-based dashboard is exposed to enable real-time visualization, data analysis, and generation of automated alerts for both clinical and technical anomalies. The goal of the demo is to highlight the system's ability to support patient monitoring in a fine-grained manner, offering a robust and extensible solution for digital health applications targeting chronic respiratory conditions.
