Invasive fungal infections (IFIs) represent an increasingly significant clinical challenge, particularly in frail or immunocompromised patients and in those undergoing invasive treatments, where they remain associated with high mortality rates. The main species involved, Candida spp. and Aspergillus spp., are responsible for complex infectious conditions that are often difficult to diagnose and treat promptly. In this context, antifungal prophylaxis in high-risk patients and targeted treatment of confirmed IFIs require optimal use of the available antifungal agents. Therapeutic drug monitoring of antifungal medications through the measurement of blood concentrations and the evaluation of pharmacokinetic/pharmacodynamic (PK/PD) targets offers a concrete opportunity to improve clinical outcomes, reduce adverse events, and limit the development of resistance. This document, prepared by the SIBioC Working Group “Therapeutic Drug Monitoring and Personalized Treatment”, aims to provide a guidance for harmonizing therapeutic ranges of the most commonly used antifungal agents, in light of current scientific evidence and clinical practice, with the goal of supporting healthcare professionals in the personalized management of antifungal therapy.
