9Tcm-DTPA renal scintigraphy after captopril in renovascular hypertension allows the detection of the decrease of glomerular filtration rate (GFR) in the kidney ipsilateral to the arterial stenosis induced by the inhibition of angiotensin II, which mediates GFR autoregulation in the lower range of renal perfusion pressure. The aim of this study was to compare this renal response with those of blood pressure and plasma renin activity in renovascular and essential hypertensives. Renovascular petients showed a marked decrease of GFR on the affected side after captopril; compared with essential hypertensives they also presented higher plasma renin increases and a greater hypotensive effect after captopril. Significant correlations were observed in the renovascular patients between the modifications of renal function after captopril and the corresponding changes of renin and blood pressure, suggesting a possible patho-physiological link between the intrarenal role of angiotensin II and the humoral and haemodynamic aspects of renovascular hypertension.
