This well conducted study, with a high response rate, addresses the issues ofeffects of prostatic biopsy in the particularly sensitive context of screening protocols.These otherwise healthy men were biopsied only for PSa elevation , without associated prostatic symptoms.As a matter of fact one of the criteriafor choice of a screening testis that it should be acceptable by thescreened population. this study reveals that prostatic biopsy was well tolerated by most patients, but associated with relevant symptomsin a minority, which in turn affected the attitude towards a repeated procedure. These observations in asymptomatic male patients undergoing prostate cancer screening represent one of the reasons why theU.S. Preventive Services Task Force recommends against it.. Within the European ERSPC screening study, sextant biopsy caused haematospermia in 54.4% of men, haematuria in 22.6% , fever in 3.5%, rehospitalization in 0.4% and prostatitis in 0.5%. In the U.S, a retrospective analuysis of the SEER prostatic biopsies data base, consisting of older subjects (mean age 73)documented a 2.65-fold increased risk of hospitalization within 30 days (6.9% vs.2.7% in controls). There is a recent trend towards an increase of infective biopsy complications, due to increased germ resistance to fluoroquinolones, and the present consensus is that local anesthetic given as periprostatic nerve block is more effective than intrarectal instillation in alleviating pain from ultrasound-guided prostate biopsy.
